Free and Reduced-Price Meals

Dubuque Community School District students may be eligible to receive free or reduced-price meals (breakfast and lunch) in accordance with guidelines set forth by the National School Lunch and School Breakfast Programs.

COMPLETE THIS FORM IF:

No member of your household is directly certified (receives federal SNAP or FIP benefits; receives a DHS letter; is homeless, a runaway or migrant; is a foster child)

AND

A member of your household attends Bryant, Carver, Eisenhower, Hempstead, Irving, Kennedy, Roosevelt, Sageville, Senior, Table Mound

A new application must be completed each and every school year unless a member of your household is directly certified.

All applications must be completed in full to be considered for eligibility.

No meal assistance form is needed if a member of your household is directly certified or no member of your household attends Bryant, Carver, Eisenhower, Hempstead, Irving, Kennedy, Roosevelt, Sageville, Senior or Table Mound.
